Job Summary
Conduct comprehensive primary care and chronic disease management for geriatric patients through in-home visits and house calls in the New York metro area. Perform in-home assessments, physical exams, and diagnostic evaluations while ordering and interpreting labs and imaging. Manage conditions such as diabetes and hypertension, prescribe medications, administer immunizations, and provide culturally-sensitive care to a majority Mandarin- or Cantonese-speaking population. Develop individualized care plans, address urgent medical needs, and ensure continuity with interdisciplinary teams and community resources.
Required Qualifications
- New York NP or PA license
- DEA
- relevant board certification (PA-C, FNP, AGNP, etc.)
- 1+ year advanced practice experience in a relevant setting
- must speak Mandarin or Cantonese fluently
Desired Qualifications
- experience with home or community-based care strongly preferred
