Job Summary
Provide mobile advanced wound care to patients in their homes or SNFs as a Nurse Practitioner or Physician Assistant in the San Jose, CA area. Responsibilities include performing wound care procedures and tests as needed, defining and documenting patient diagnoses, ensuring patient satisfaction, patient education and management, debridement of wounds, application of topical wound care products, and documenting findings and follow-up in the patient chart. Requires CA NP/PA license and CPR certification, valid driver’s license, and willingness to drive to patient homes; extensive training and 24/7 support provided; on-the-road, non-remote role.
Required Qualifications
- Licensed NP or PA in CA
- Current state license as NP or PA in CA
- CPR Certification
- Valid driver’s license
- Minimum state required auto insurance
- Willing to drive to patient homes within designated region
