Nurse Practitioner / Physician Assistant needed to perform H&Ps on CDPAP-registered caregivers (adult patients) in Suffolk County, NY. Provides professional nursing care per established standards, coordinates with the care team to ensure coordinated, patient-centered care, and directs licensed practical nurses, clerical staff, and medical assistants as needed. Role supports a flexible schedule with the ability to choose days; typical tasks include brief physical assessments (8-10 minutes for H&P), and functioning in an ambulatory medical-office environment with exposure to communicable diseases and occasional lifting. Minimum qualification: NY-licensed NP or PA. Preferred: at least two years of recent clinical nursing experience in a hospital, medical office, or similar setting.
