Job Summary
Nurse Practitioner or Physician Assistant provides primary and palliative home-care to patients in the Charleston, SC region, performing physical examinations, updating medical histories, developing care plans, ordering diagnostic testing, and prescribing medically necessary interventions under physician supervision. Responsibilities include delivering high-quality, patient-centered care to a panel of homebound patients, communicating with office staff and clinical operations, attending quarterly medical staff meetings, and collaborating with patients, families, and caregivers. Requires board certification with valid SC NP/PA certification, 1-2 years of primary care experience with geriatric patients (hospice experience preferred), and the ability to work independently in a flexible schedule with support from an in-house clinical team and opportunities for growth into leadership roles. Required Qualifications
- Board Certified, with a valid NP or PA certification in the state of South Carolina
- Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
- Positive demeanor and strong people skills
- Ability to confidently problem solve and operate independently
- 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)
