Job Summary
Provide comprehensive care to seniors following clinical standards while collaborating daily with a multidisciplinary team to ensure coordinated, high-quality outcomes. Manage referrals, hospital/SNF coordination, DME, and home health services, and document care accurately with support from quality-based coders. Participate in on-call rotation and support growth plans at the Southside clinic in Corpus Christi, Texas. This role offers 40 weekly hours with a base pay range of $123,800 - $170,300, plus a bonus incentive plan. Candidates require a Master's degree or PA program completion with board certification and an active, unrestricted license in the state of practice.
Required Qualifications
- Master's Degree in Nursing or completion of a PA program with board certification
- active, unrestricted NP or PA license in the state of practice
- ability to obtain DEA license and/or Medicaid and Medicare numbers
Desired Qualifications
- 2 or more years of experience in value-based care, primary care or Medicare Advantage settings
- active DEA license
- Medicare/Medicaid provider numbers
