Job Summary
Conduct comprehensive primary care services focusing on prevention, chronic disease management, and health promotion for diverse patient populations in an outpatient setting. Diagnose and treat acute and chronic conditions using evidence-based guidelines, prescribe medications, and refer to specialists when necessary. Collaborate with physicians, nurses, pharmacists, and care coordinators to optimize community health, participate in quality improvement initiatives, and support transitions of care to reduce readmissions. Ensure timely documentation of direct and asynchronous encounters while delivering risk assessment and preventive oversight for high-risk populations.
Required Qualifications
- Active Washington State Nurse Practitioner or Physician Assistant license
- DEA and state prescriptive authority
- Current BLS for Healthcare Providers certification by the American Heart Association
Desired Qualifications
- Minimum of two years' experience as a primary care provider
