Nurse Practitioner - Physician Assistant

Job Summary

Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular or pulmonary conditions while developing individualized treatment plans. Order, interpret, and communicate laboratory studies, imaging, and other diagnostic testing, then prescribe medications within your scope of practice. Educate patients, families, and caregivers regarding diagnoses, treatment plans, medications, and ongoing care while coordinating with physicians, nursing staff, therapy, pharmacy, and social services. Round at assigned Skilled Nursing, Long-Term Care, and Post-Acute Care facilities in Tinley Park, Blue Island, Oak Lawn, and surrounding communities. Maintain accurate, timely, and compliant clinical documentation. This role requires a current, unrestricted Nurse Practitioner or Physician Assistant license, national certification, BLS certification, and at least six months of clinical experience. Full-time daytime Monday through Friday schedule with no regularly scheduled weekends or traditional on-call responsibilities.

Required Qualifications

  • Current, unrestricted Nurse Practitioner or Physician Assistant license in the applicable state(s) of practice.
  • Current national certification appropriate to licensure.
  • Current Basic Life Support (BLS) certification.
  • Minimum six (6) months of Nurse Practitioner or Physician Assistant clinical experience.
  • Valid driver's license with reliable transportation.
  • Ability to travel daily throughout an assigned territory.
  • Strong clinical judgment, communication, organizational, and critical thinking skills.
  • Experience utilizing Electronic Medical Records (EMR).

Desired Qualifications

  • Two (2) or more years of Nurse Practitioner or Physician Assistant experience.
  • Experience in Skilled Nursing, Long-Term Care, Post-Acute Care, Internal Medicine, Cardiology, Pulmonology, Hospital Medicine, or Geriatrics.
  • Experience caring for medically complex patient populations.
