Job Summary
Conduct comprehensive health assessments, perform medical diagnoses of acute and chronic illnesses, and formulate treatment plans for patients in home health and hospice settings. Interview patients, employees, and family members to construct medical histories, evaluate patient behavior, and discuss health needs. Provide patient education on diagnosis, treatment, prognosis, and preventive care while securing informed consent and administering medications or vaccines. Directly perform or order diagnostic studies, interpret results, and follow up on abnormal tests. Maintain accurate documentation of patient care, medication records, and discharge information while using cost-containment measures for prescriptions and referrals. Work under the scope of practice of a nurse practitioner and as an agent of the attending physician.
Required Qualifications
- Master's Degree in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Registered Nurse (RN) licensure in the state of practice
- Nurse Practitioner (NP) licensure in state of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- all required vaccinations as a condition of employment and annually thereafter, where applicable
- acknowledged access to and reading of the E-Verify Participation and Right to Work notices
