Job Summary
Evaluate and treat patients in hospice, palliative, and home health programs while collaborating with the interdisciplinary team and medical director. Perform physical exams, formulate differential diagnoses, and determine clinical appropriateness for admissions, transfers, and discharges. Assess individual needs for pain management and symptom control, participate in Plan of Care development, and write orders for medications and treatments. Order and interpret laboratory and diagnostic tests, conduct face-to-face recertification visits, and prepare accurate patient records for coding and reimbursement. Provide ongoing support to clinical staff and work with the HIM department to ensure documentation authorization.
Required Qualifications
- Graduate from a nationally accredited educational program as an advanced practitioner of nursing, or adult, family or geriatric nurse practitioner
- One to two (1-2) years of experience as a Nurse Practitioner
- Current Colorado RN and APN/NP licenses
- Board certified as Adult or Family Nurse Practitioner by the AANP or ANCC
- Valid Colorado state driver's license
- Prescriptive authority and DEA number
- Current CPR certification
- Must be able to obtain hospital privileges at all local hospitals
- Ability to lift minimum of 50 lbs
- Monday-Friday, 8-5, on-call rotating basis
