Job Summary
APRN/PA role focused on geriatrics in long-term care facilities, providing personalized, coordinated healthcare to residents enrolled in Medicare Advantage plans. Responsibilities include assessing medical and mental health needs, creating and implementing plans of care, prescribing medications, ordering labs and consultations, monitoring treatment responses, ensuring regulatory compliance, and collaborating with RN case managers and facility leadership to implement our model of care.
Required Qualifications
- FNP, AGNP, AHACNP or PA license required
- Degree from an accredited APRN or PA program
- 3 years’ experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
