Job Summary
Advanced practice provider (NP/PA) in geriatrics/primary care working in long-term care settings, collaborating with a team of providers and case managers to deliver personalized, coordinated care for senior residents enrolled in Medicare Advantage plans. Responsibilities include assessing patients’ medical and mental health needs, developing and updating care plans, prescribing medications and ordering labs/diagnostics, monitoring treatment responses, ensuring compliance with local/state/federal regulations, and coordinating with RN case managers and facility staff to improve quality of life and reduce unnecessary hospital visits. Requires NP/PA license with degree from an accredited APRN/PA program; experience in geriatric populations; EHR proficiency; and ability to work within an integrated care team.
Required Qualifications
- FNP, AGNP, AHACNP or PA license required
- Degree from an accredited APRN or PA program
- 3 years’ experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
