Job Summary
Provide advanced practice care as a Nurse Practitioner or Physician Assistant with geriatrics/primary care focus in long-term care settings (nursing homes, assisted living) within TruHealth/American Health Partners. Independently manage patient needs with supervision as appropriate, develop plans of care, prescribe medications, order labs and diagnostics, monitor treatment response, and coordinate with RN case managers and health-plan leadership to implement a national care model compliant with federal/state regulations. Responsibilities include assessing medical and mental health needs, ensuring regulatory compliance, and collaborating with facility staff. Requires NP/PA license and graduated from an accredited APRN/PA program, plus 3 years of geriatric clinical experience and EHR proficiency. Part-time role with potential for growth; experience with Medicare Advantage plans and geriatrics preferred.
Required Qualifications
- FNP, AGNP, AHACNP or PA license required
- Degree from an accredited APRN or PA program
- 3 years’ experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
