Job Summary
Nurse Practitioner in a Part-Time role at designated skilled nursing facilities within Provider Partner Health Plan, collaborating with the member’s primary care physician to deliver preventive, acute, and chronic care. Serves as medical liaison to the interdisciplinary care team, communicates effectively with members, families, and staff, and develops care plans with short- and long-term goals. Responsibilities include comprehensive health assessments, ordering and interpreting diagnostic tests, prescribing medications, referring to specialists when appropriate, and ensuring adherence to PPHP standards of documentation and care transitions. Requires independent thinking, autonomy, and strong interpersonal skills.
Required Qualifications
- Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice.
- Meets the Provider Partner Health Plan credentialing requirements.
- Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice).
- Basic Cardiac Life Support certification.
- Current unrestricted RN license for the state in which the Nurse Practitioner is practicing.
- Valid driver’s license and car insurance with reliable transportation.
- Minimum of 3 years clinical care experience.
- Nursing experience in Skilled Nursing Facility/Long Term Care preferred.
- Case management experience preferred.
- Knowledge of Medicare/Medicaid.
- Detailed knowledge of I-SNP MOC and I-SNP population.
- Proficiency in nursing/medical databases and Microsoft programs.
- Positive and enthusiastic attitude.
