Job Summary
Assess patients' medical and mental health needs and provide care plans for long-term care residents enrolled in Medicare Advantage plans. Prescribe medications, order lab work and diagnostic procedures, and monitor treatment response while modifying plans as necessary. Ensure compliance with local, state, and federal agencies regarding clinical services and consult with a supervising physician for acute and chronic conditions. Collaborate with RN case managers and facility partners to implement the national model of care. Provide training and advice to facility staff as needed.
Required Qualifications
- FNP, AGNP, AHACNP or PA license
- Degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
- Masters or better
- APRN Advanced Practice RN
