Job Summary
Assess patients' medical and mental health needs to provide care plans for long-term care residents, while prescribing medications, ordering labs, and monitoring treatment response. Exercise independent clinical judgment to treat acute and chronic conditions, collaborating with RN case managers and facility partners to implement the national model of care. Ensure compliance with local, state, and federal regulations for all clinical services provided. This role supports TruHealth's innovative, proactive model that promotes dignity and independence for seniors enrolled in Medicare Advantage plans.
Required Qualifications
- FNP, AGNP, AHACNP or PA license
- Degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
- Masters or better
- APRN Advanced Practice RN
- Primary Care in Geriatric Medicine
- 3 year(s): Long Term Care
Desired Qualifications
- experience in primary care or internal medicine, preferably with geriatrics focus
