Job Summary
Assess patients' medical and mental health needs, prescribe medications, order lab work and diagnostic procedures, and modify treatment plans based on monitoring compliance and response. Provide personalized, coordinated health care to long-term care residents enrolled in Medicare Advantage plans, preventing unnecessary hospital visits. Exercise independent judgment to treat acute and chronic conditions while consulting a supervising physician as appropriate. Ensure compliance with local, state, and federal agencies and implement the national model of care in collaboration with facility partners and case managers. May provide training and advice to facility staff.
Required Qualifications
- FNP, AGNP, AHACNP or PA license
- Degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
- APRN Advanced Practice RN
- Masters or better
