Job Summary
Assess patients' medical and mental health needs and provide a plan of care that proactively manages conditions and addresses barriers to care. Prescribe medications, order lab work, diagnostic procedures, and consultations while monitoring patient compliance and modifying treatment plans as needed. Ensure compliance with local, state, and federal agencies and work closely with RN case managers to implement the national model of care in compliance with all regulations. Exercise independent judgment to treat residents with acute and chronic conditions in long-term care facilities.
Required Qualifications
- FNP, AGNP, AHACNP or PA license
- Degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
- Masters or better
- APRN Advanced Practice RN
- Primary Care in Geriatric Medicine
- 3 year(s): Long Term Care
