Job Summary
Assess patients' medical and mental health needs, prescribe medications, order lab work, and modify treatment plans for long-term care residents. Provide personalized, coordinated health care that improves quality of life and prevents unnecessary hospital visits while ensuring compliance with local, state, and federal agencies. Monitor patient response to treatment and work closely with RN case managers to implement the national model of care in compliance with all regulations. Exercise independent judgment to treat acute and chronic conditions, consulting with a supervising physician as appropriate.
Required Qualifications
- FNP, AGNP, AHACNP or PA license
- Degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
- APRN Advanced Practice RN
- Masters or better
