Job Summary
Assess patients' medical and mental health needs and provide a plan of care that proactively manages conditions and addresses barriers to treatment. Prescribe medications, order lab work, diagnostic procedures, and consultations while monitoring patient compliance and modifying plans as necessary. Ensure compliance with local, state, and federal agencies regarding clinical services. Work closely with RN case managers as part of an integrated care team to implement the national model of care in compliance with regulations. Exercise independent judgment to treat acute and chronic conditions, consulting with a supervising physician as appropriate. Provide training and advice to facility staff.
Required Qualifications
- FNP, AGNP, AHACNP or PA license
- Degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
- Masters or better
- APRN Advanced Practice RN
- Primary Care in Geriatric Medicine
- 3 year(s): Long Term Care
Desired Qualifications
- experience in primary care or internal medicine, preferably with geriatrics focus
