Job Summary
Assess patients' medical and mental health needs and provide a plan of care that proactively manages conditions and addresses barriers to care. Prescribe medications, order lab work, diagnostic procedures, and consultations while monitoring treatment compliance and modifying plans as needed. Ensure compliance with local, state, and federal agencies regarding clinical services. Work closely with RN case managers and facility partners to implement the national model of care in compliance with regulations. Exercise independent judgment to treat acute and chronic conditions, consulting a supervising physician as appropriate.
Required Qualifications
- FNP, AGNP, AHACNP or PA license
- Degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
- APRN Advanced Practice RN
- Masters or better
