Nurse Practitioner- Part-Time

Assess patients' medical and mental health needs and provide a plan of care that proactively manages conditions and addresses barriers to treatment. Prescribe medications, order lab work, and monitor patient response while modifying plans as necessary. Ensure compliance with local, state, and federal agencies related to clinical services. Collaborate with RN case managers and facility partners to implement the national model of care in compliance with regulations. Exercise independent judgment to treat residents with acute and chronic conditions, consulting with a supervising physician as appropriate.
