Job Summary
Conduct comprehensive hospice services in compliance with the state Nurse Practice Act under physician supervision. Evaluate health history, diagnose medical conditions, and develop individualized plans of care while prescribing medications and treatments. Perform face-to-face assessments to assess hospice eligibility and recertification, coordinating services with the interdisciplinary team and community resources. Manage terminal illnesses by controlling symptoms and reviewing clinical protocols in collaboration with the medical director. Maintain accurate documentation, counsel patients and families on end-of-life issues, and participate in quality improvement initiatives.
Required Qualifications
- Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program
- Current DEA license
- Current nursing licensure in State
- CPR certification
- Required licensure, education, training, and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state
- Accreditation from an approved certifying body for advanced practice nursing as required by State
- Previous home care/hospice experience
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Ability to lift, position and/or transfer patients
- Ability to lift supplies and equipment
- Visual acuity and hearing to perform required nursing skills
- Prolonged or considerable walking or standing capability
Desired Qualifications
- Certification in a specialist area (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
