Job Summary
Facilitate comprehensive, individualized palliative care treatment plans for patients, providing diagnosis, treatment, consultation, and follow-up under the direction of the Palliative Medical Director. Deliver care ensuring excellent clinical skills and ethical judgment while managing Medicare/Medicaid regulations and value-based care models. Requires Master of Science in Nursing, two years of clinical experience, and proficiency with electronic medical record software. Schedule availability is 1-2 days per week with 10-20% travel for training.
Required Qualifications
- Nurse Practitioner Licensure - MN
- DEA Licensure
- Schedule Availabilty - 1-2 days/week
- Meets continuing education requirements set by Gentiva and licensing boards
- Master of Science in Nursing (MSN) or equivalent from an accredited institution
- Minimum two years of Nurse Practitioner clinical experience
- Current RN license or equivalent as required by state law
- Active and Unrestricted Nurse Practitioner certification/license without board actions
- Collaborative agreement with supervising physician from the Professional Services Corporation
- Part B Provider certification
- Federal DEA license; state DEA license if applicable
- Valid driver's license and current automobile insurance
- Reliable transportation required
- Willingness to travel 10-20% for training and education
Desired Qualifications
- Palliative Care or Hospice Care experience preferred
- Specialty in Family or Adult Medicine preferred
- Experience with value-based care models and Part B billing preferred
- Ability to work autonomously in an advanced practice role
- ACHPN certification preferred
