Nurse Practitioner Palliative Care

Job Summary
Collaborate with the Palliative Medical Director to deliver comprehensive, patient-centered palliative care to patients in their homes. Perform in-depth physical, emotional, and psychosocial assessments, then develop and implement individualized care plans in partnership with patients and families. Manage treatment plans, prescribe medications including controlled substances, and order diagnostic tests as permitted by state law. Maintain accurate clinical documentation and communicate effectively with physicians, caregivers, and other healthcare professionals. Participate in evening/weekend call rotation as needed. Requires Master of Science in Nursing, two years of NP experience, and current RN and NP licensure. Part B billing and DEA licenses are mandatory; ACHPN certification preferred.
Required Qualifications
- Master of Science in Nursing from an accredited school of nursing (or equivalent per state law)
- Minimum two years' Nurse Practitioner experience
- Current RN license and Nurse Practitioner certification/license as required by state law
- Collaborative agreement with supervising physician (or willingness to establish)
- Part B Provider certification
- Federal DEA license and state DEA license (if required)
- Valid driver's license and current automobile insurance
- Meets mandatory continuing education requirements for Gentiva and licensing boards
- Proficient in point-of-care software and technology
- Ability to work autonomously in an advanced practice role
- Understanding of palliative medicine principles, home health, hospice care eligibility, and Medicare/Medicaid regulations
- Familiarity with the interdisciplinary care process
- Excellent leadership, interpersonal, written, and verbal communication skills
- Strong independent decision-making abilities and self-motivation
Desired Qualifications
- Specialty in Family or Adult Medicine preferred
- Experience with Part B billing
- ACHPN certification in Hospice and Palliative Care preferred
