Nurse Practitioner Palliative Care

Job Summary

Conduct comprehensive, patient-centered palliative care for patients in their homes, performing in-depth physical, emotional, and psychosocial assessments. Develop and implement individualized care plans while managing treatment adjustments, prescribing medications including controlled substances, and ordering diagnostic tests. Collaborate with the Palliative Medical Director and interdisciplinary team to guide patients and families through every stage of treatment, maintaining accurate clinical documentation and facilitating referrals for additional services. Participate in evening/weekend call rotation as needed. Work autonomously in an advanced practice role supporting Empatia's mission to provide compassionate care across nearly 600 locations.

Required Qualifications

  • Master of Science in Nursing from an accredited school of nursing (or equivalent per state law)
  • Minimum two years' Nurse Practitioner experience
  • Current RN license and Nurse Practitioner certification/license as required by state law
  • Collaborative agreement with supervising physician (or willingness to establish)
  • Part B Provider certification
  • Federal DEA license and state DEA license (if required)
  • Valid driver's license and current automobile insurance
  • Meets mandatory continuing education requirements for Gentiva and licensing boards
  • Proficient in point-of-care software and technology
  • Ability to work autonomously in an advanced practice role
  • Understanding of palliative medicine principles, home health, hospice care eligibility, and Medicare/Medicaid regulations
  • Familiarity with the interdisciplinary care process
  • Excellent leadership, interpersonal, written, and verbal communication skills
  • Strong independent decision-making abilities and self-motivation

Desired Qualifications

  • Specialty in Family or Adult Medicine preferred
  • Experience with Part B billing
  • ACHPN certification in Hospice and Palliative Care preferred
