Job Summary
Conduct comprehensive specialized care for senior living residents, including serious illness conversations, advanced care planning, and expert symptom management. Perform complete physical exams, interpret diagnostic data, and determine treatment plans while independently managing acute and long-term medical needs. Facilitate goals of care discussions, guide families through advance directives and POLST forms, and prescribe pharmacologic and non-pharmacologic treatments. Collaborate with Curana primary teams and facility staff to ensure seamless care continuity and provide education on palliative care principles. Based within multiple facilities with travel between locations within driving distance.
Required Qualifications
- current state RN and NP licensure
- current Board Certification in a Nursing Specialty/Field
- minimum of a Bachelors and Masters Degree in Nursing from accredited Colleges or Universities
- knowledge of Nursing and clinical Nurse Practitioner critical thinking skills application to the aged, chronically ill, and convalescent resident
- excellent written and oral communication skills
- traits of punctuality, organizational skills, and ability to function within a team
- Location: Based within multiple senior living facilities with travel between facilities within driving distance
Desired Qualifications
- Minimum of 2 years of Nurse Practitioner experience
- palliative care or hospice background
- ACHPN certification
- willingness to complete ACHPN certification within a year of employment
- Strong leadership, interpersonal, and problem-solving skills
