Job Summary
Conduct comprehensive specialized care for senior living residents in long-term, assisted, and independent living settings, focusing on serious illness conversations, advanced care planning, and expert symptom management. Perform complete physical exams, interpret diagnostic data, and determine treatment plans for chronically ill patients. Facilitate goals of care discussions with patients and families, guide residents in completing advance directives and POLST forms, and manage acute and long-term medical needs. Prescribe pharmacologic and non-pharmacologic treatments, order diagnostic tests, and maintain accurate patient records. Collaborate with Curana primary teams and facility staff to ensure no gaps in care while educating partners on palliative care principles. Location: Based within multiple senior living facilities with travel between facilities within driving distance.
Required Qualifications
- current state RN and NP licensure
- current Board Certification in a Nursing Specialty/Field
- minimum of a Bachelors and Masters Degree in Nursing from accredited Colleges or Universities
- knowledge of Nursing and clinical Nurse Practitioner critical thinking skills application to the aged, chronically ill, and convalescent resident
- excellent written and oral communication skills
- traits of punctuality, organizational skills, and ability to function within a team
Desired Qualifications
- minimum of 2 years of Nurse Practitioner experience
- palliative care or hospice background
- ACHPN certification
- willingness to complete ACHPN certification within a year of employment
