Job Summary
Nurse Practitioner – Palliative & Primary Care providing primary and palliative care with emphasis on symptom management, quality of life, and goal-concordant care. Manage comprehensive history and physicals for patients with complex illnesses; lead goals-of-care discussions, advance directives, POLST/DNR orders; address pain and non-pain symptoms using pharmacologic and non-pharmacologic approaches; collaborate with an interdisciplinary team to review patient status and care plans; facilitate timely transitions of care to hospice or other services; educate patients and families about disease trajectory, prognosis, and end-of-life expectations; participate in readmission reviews and optimize symptom control to prevent avoidable hospitalizations; provide clinical resources and ongoing care coordination with nursing, social work, chaplaincy, and therapy services; works within a full-time benefits package with license/certification reimbursement and ample PTO.
Required Qualifications
- Master’s (MSN) or Doctorate (DNP) Nurse Practitioner program completion
- Active Nurse Practitioner license
- Experience in primary care, geriatrics, or post-acute care preferred but not required
- Palliative care training provided
