Job Summary
Provide primary and palliative care services focused on symptom management, quality of life, and goal-concordant care; lead and document goals-of-care discussions including advance care planning and code status conversations; facilitate completion and updates of advance directives (POLST/DNR) and end-of-life preferences; manage pain and non-pain symptoms with pharmacologic and non-pharmacologic approaches; review vitals and labs, escalate as needed; evaluate and manage new admissions ensuring alignment with patient goals; participate in readmission reviews to improve symptom control and prevent avoidable hospitalizations; collaborate with the interdisciplinary team weekly to review status and care plans; identify patients appropriate for palliative or hospice referral and support care transitions; provide education and emotional support to patients and families regarding disease trajectory and prognosis; collaborate with nursing, social work, chaplaincy, and therapy services; monitor intervention effectiveness and adjust plans to maintain comfort and dignity; serve as a clinical resource on palliative care principles for facility staff; strong benefits package including medical/dental/vision, 401(k), PTO, and license/certification reimbursement.
Required Qualifications
- Master’s (MSN) or Doctorate (DNP) Nurse Practitioner program completion
- Experience in primary care, geriatrics, or post-acute care preferred, but not required
- Palliative care training and certification provided
