Job Summary
Provide primary and palliative care services focusing on symptom management, quality of life, and goal-concordant care; conduct comprehensive histories and exams for patients with complex illnesses; lead goals-of-care discussions including advance care planning and code status; facilitate advance directives and POLST/DNR orders; manage physical and non-physical symptoms; review vitals and labs and escalate as needed; evaluate admissions and align treatment plans with patient goals; participate in IDT weekly to review status and plans; identify patients suitable for palliative or hospice referral and coordinate transitions of care; support care transitions with emphasis on continuity, comfort, and patient-centered goals; educate and support patients and families regarding disease trajectory and end-of-life expectations; collaborate with nursing, social work, chaplaincy, and therapy services; monitor interventions and adjust plans to maintain comfort and dignity; serve as a clinical resource on palliative care principles; position located in Dover, NJ within a full‐time capacity.
Required Qualifications
- Master’s (MSN) or Doctorate (DNP) Nurse Practitioner program completion
- Palliative care training and certification provided
