Job Summary
Nurse Practitioner provides patient assessment, collaborates with physicians and health professionals to prepare comprehensive patient care plans in hospice/home health settings, prescribes or recommends drugs or other treatments, and monitors patient progress. Works with hospice IDT and other health professionals to ensure coordinated care; requires NP program completion and state licensure, and at least 2 years of hospice/home health or community health experience. Day shift, full-time role in Fort Myers, FL.
Required Qualifications
- Minimum of 2 years experience in hospice, home health or community health in the past five years
- Completion of accredited Nurse Practitioner program
- State licensure required
- Lifting requirements: assist in lifting and transferring patients; lift objects in excess of 20 pounds
- Physical requirements: standing or walking >1 hour/day; ability to talk, hear, sit, stoop, kneel, crouch as needed
