Job Summary

Provide primary care to an assigned panel of SCAN members in their place of residence, including senior living communities and skilled nursing facilities. Conduct physical and wellness exams, administer vaccines, and interpret diagnostic tests while managing chronic disease conditions like dementia and diabetes. Collaborate with case management and primary care physicians to develop comprehensive care plans, initiate goals of care conversations, and ensure timely access to necessary services. Independently manage visitation schedules based on member acuity and participate in on-call rotations for after-hours and weekend support. Document visits according to established protocols and engage with the sales team to educate communities. Maintain professional knowledge through workshops and support team efforts to close care gaps and optimize clinical workflows.

Required Qualifications

  • Master's degree in Nurse Practitioner Certificate OR Master's from an accredited PA program
  • Current Arizona Nurse Practitioner license OR Arizona Physician Assistant Certification (PA-C) in good standing
  • Valid driver's license
  • Automobile insurance
  • Reliable transportation
  • Ability to stoop, kneel, and crouch
  • Ability to bend, reach, lift, pull, push, and carry up to 10 lbs. regularly and up to 50 lbs. occasionally
  • Travel 75+% of the time in the assigned area
  • Proof of Tuberculosis screening upon hire or proof of a negative screening within the last year
  • Background check

Desired Qualifications

  • Education in Adult/Geriatric medical care
  • Wound Care Certificate
  • Palliative Care Certificate
  • Knowledge of the treatment of complex chronic diseases, such as dementia, diabetes, heart failure, and COPD
  • Knowledge of HCC and ICD-10 coding and documentation
  • Knowledge of wound care and management
  • Knowledge of CMS guidelines and Medicare Advantage Managed Care
  • Comfortable with initiating goals of care and end of life discussions
  • Strong interpersonal skills and change agent mindset needed to build relationships with healthcare team and patients
  • Knowledge of and ability to maintain HIPAA requirements
  • Adaptable and open to change and problem-solving
  • Knowledge of and/or ability to learn basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and open to new technology
  • Multilingual/Multicultural
