Job Summary
Experienced Nurse Practitioner or Physician Assistant sought to provide primary and palliative home-based care in Sussex County, NJ. Responsibilities include performing physical examinations, reviewing medical histories, developing care plans, diagnosing and treating chronic conditions, prescribing medications and interventions, ordering diagnostic tests, and coordinating with office staff and the clinical team. Attend quarterly medical staff meetings and provide patient- and family-centered care, with emphasis on geriatric populations. Requires NJ-licensed NP or PA certification and board certification, 1-2 years of primary care experience (hospice experience preferred).
