Job Summary
Conduct comprehensive geriatric assessments, diagnoses, and treatment plans for complex Medicare patients with multimorbidity. Manage acute and chronic conditions through a blend of direct patient care and care management, including panel management, care coordination, and medication monitoring for polypharmacy. Perform in-basket management for communications and results review, while supporting transitions of care between hospitals and SNFs. Deliver preventive screenings and close care gaps within a dyad partnership with geriatric physicians, utilizing in-clinic, home-based, and virtual delivery models. Collaborate with RNs, social workers, pharmacists, and specialists to improve quality of life for older adults. Requires current California NP or PA license, CAPA certification, and active DEA registration.
Required Qualifications
- Current California State NP or PA License
- CAPA Controlled Substance Certification (for Physician Assistants only)
- Current Nurse Practitioner Furnishing Number (for Nurse Practitioner only)
- Active DEA registration
Desired Qualifications
- Minimum 2-3 years of clinical experience preferred
- geriatrics or complex care experience strongly preferred
- Experience with high-risk senior populations or panel-based care
- Familiarity with Epic or other EHR systems
- Experience with care management, transitions of care, or home-based care delivery
