Job Summary
Conduct comprehensive patient assessments, including medical history, physical exams, and diagnostic tests for high-risk populations. Develop and implement treatment plans, prescribe medications, and provide patient education on health maintenance and goals of care. Collaborate with physicians and nurses in interdisciplinary team meetings to ensure coordinated care and monitor patient outcomes for quality improvement. Maintain accurate documentation and stay current with medical advancements through ongoing education. Work remotely and in the field approximately 75% of the time in Riverside, CA, supporting SCAN's mission to keep seniors healthy and independent.
Required Qualifications
- Completion of an accredited Advanced Practice Clinician program
- CA Registered Nurse (RN)
- 3-5 years of related experience
- Travel 75+% of the time in the assigned area
- Must have a valid driver's license, automobile insurance and reliable transportation
- Good communication and interpersonal skills
- Strong knowledge of clinical practices and procedures
- Ability to independently diagnose and treat common chronic diseases
- Ability to analyze complex patient data and make informed clinical decisions
- Strong problem-solving skills to address patient care challenges
- Ability to work collaboratively with a multidisciplinary healthcare team
- Strong interpersonal skills to build effective relationships with patients and colleagues
- Proficiency in electronic health record (EHR) systems and other clinical software
- Ability to use technology to enhance patient care and documentation
- Ability to manage a busy caseload and prioritize tasks effectively
- Strong organizational skills to ensure timely and efficient patient care
- Ability to maintain accurate and detailed patient records
- Meticulous in following clinical protocols and procedures
