Job Summary
Nurse Practitioner or Physician Assistant needed to perform in-home Comprehensive Health Assessments (CHA) for adults/geriatric patients, with visits up to one hour at members' homes or Skilled Nursing Facilities. Responsibilities include reviewing medical history, health status, medication adherence, social environment, and risks; administering point-of-care and lab testing; educating patients; coordinating with PCP and care team; and documenting per Matrix guidelines. Role requires collaboration with physicians, case managers, social workers, and family caregivers; travel to members’ residences; flexible scheduling (8am–7pm, Sunday–Saturday). Full-time opportunities with potential transition to FT and a $2,000 sign-on bonus; ongoing support for licensure/credentialing; benefits include mileage reimbursement, 401(k), and continuing education access.
Required Qualifications
- Master’s Degree or commensurate experience and NP licensure
- Current NP licensure in state of practice with prescription authority or ability to obtain
- AANP, AACN, or ANCC board certification in applicable specialty
- BLS, ACLS or CPR certification
- 1 year NP experience preferred; new grads encouraged
- Valid state driver’s license and automobile insurance as required
