Job Summary
Conduct adult and geriatric health assessments at member residences or skilled nursing facilities, reviewing medical history, medication adherence, and social risks while administering point-of-care and lab-based testing. Collaborate with physicians, case managers, and caregivers to manage referrals, provide health education, and ensure clear documentation per Matrix guidelines. Travel to see members outside your home location to increase assessment volume and earn income based on completed visits.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice
- Prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
Desired Qualifications
- 1 year of NP experience preferred
