Nurse Practitioner or Physician Assistant (PRN) conducting in-home Comprehensive Health Assessments for adults/geriatric patients, reviewing medical history, medications, and social environment with up to one-hour visits at members’ residences or Skilled Nursing Facilities. Responsibilities include medical history intake, diagnosis and treatment documentation, health education, physician and case-management referrals, ordering point-of-care and lab testing, collaboration with PCP and other care team members, and ensuring clear documentation per Matrix guidelines. Role emphasizes traveling to member homes, flexible scheduling (including weekends), and potential transition to full-time roles; requires NP licensure with prescriptive authority (or ability to obtain) and board certification in a relevant specialty, along with licensure and driving requirements.
