Job Summary
Conduct adult and geriatric health assessments in members' homes or skilled nursing facilities, reviewing medical history, medication adherence, and social risks while administering point-of-care and lab-based testing. Collaborate with primary care physicians, case managers, and caregivers to manage referrals, provide health education, and ensure clear documentation per Matrix protocols. Schedule visits between 8 am and 7 pm, Monday through Saturday, based on personal availability, with compensation tied to the number of completed assessments ranging from $90 to $115 per visit. Master's degree or commensurate experience with current NP licensure and BLS/ACLS certification required.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
