Job Summary
PRN Nurse Practitioner or Physician Assistant conducting in-home health assessments (CHA) at member residences, including medical history review, medication adherence, social environment and other risks; administer and order point-of-care and lab-based testing; collaborate with physicians, case managers, social workers, and caregivers; provide services at members’ homes or Skilled Nursing Facilities; flexible scheduling Sundays– Saturdays 8am–7pm; potential transition to full-time; earn $1,800–$11,500 per month (annualized to $21,600–$138,000); mileage reimbursement and licensure support for additional states; strong emphasis on patient education, documentation, and care coordination; requires Master’s or commensurate NP licensure and board certification, with prescriptive authority and CPR/BLS/ACLS; travel required to member residences.
Required Qualifications
- Master’s Degree or commensurate NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP experience preferred, new grads encouraged to apply!
- Valid state driver’s license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
