Job Summary
Provide in-home Comprehensive Health Assessments for adults/geriatric members at the member’s residence or Skilled Nursing Facilities, reviewing medical history, diagnoses, treatment, medications, and social environment to identify health risks and close care gaps. Administer and order point-of-care and laboratory testing as needed. Collaborate with physicians, case managers, social workers, family members, and other care team members. Educate patients, coordinate follow-up with PCP, and document findings per Matrix guidelines. Eligible providers may work flexible schedules (PRN) with potential transition to full-time roles, with per-assessment compensation driving earning potential (monthly range $1,800–$11,500) and mileage reimbursement. Requires NP licensure, board certification in a relevant specialty, CPR/BLS or ACLS, and a valid driver’s license; compensation includes benefits and ongoing education support.
Required Qualifications
- Master’s Degree or commensurate experience and NP licensure
- Current NP licensure in state of practice with prescription authority or ability to obtain prescriptive authority
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP experience preferred, new grads encouraged to apply
- Valid state driver‘s license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
