Nurse Practitioner or Physician Assistant (PRN) - In-Home Health Assessments

Nurse Practitioner or Physician Assistant (PRN) conducting in-home Comprehensive Health Assessments. Responsibilities include reviewing a member’s health, medical history, medication adherence, social environment and other risks during visits that can last up to one hour; performing adult/geriatric assessments with documentation per Matrix guidelines; administering and ordering point-of-care and lab-based testing; collaborating with physicians, case managers, social workers, and caregivers; providing patient education and follow-up; and delivering services at the member’s residence, including homes or Skilled Nursing Facilities. Opportunities offer flexible scheduling (including Sundays– Saturdays 8am–7pm) with potential transition to full-time roles and a sign-on bonus, and compensation tied to the number of assessments completed.
