Job Summary
Nurse Practitioner or Physician Assistant (PRN) conducting in-home Comprehensive Health Assessments (CHA) for adults/geriatric members. Responsibilities include reviewing medical history, medication adherence, social/environmental factors, and providing health education, referrals (physician, case management), and clear documentation; administering point-of-care and lab tests as needed; coordinating with physicians, case managers, social workers, and caregivers; delivering services at the member’s home or in Skilled Nursing Facilities; flexible scheduling with weekend availability; compensation based on number of assessments with monthly earnings potential and a $2,000 sign-on bonus; opportunities to transition to full-time. Requires NP licensure and prescriptive authority, board certification, BLS/CPR, and strong communication and computer skills.
Required Qualifications
- Master’s Degree or commensurate experience and satisfactory NP licensure
- Current NP licensure in state of practice including prescriptive authority or ability to obtain
- Board certification in Adult, Family, or Gerontology specialties (AANP, AACN, ANCC)
- BLS, ACLS or CPR certification
- 1 year NP experience preferred; new grads encouraged to apply
- Valid state driver’s license and vehicle insurance in residence state
- Strong computer skills and familiarity with EMR/health records software
- Excellent verbal and written communication skills
