Job Summary
Conduct adult and geriatric health assessments in members' homes or Skilled Nursing Facilities, reviewing medical history, medication adherence, and social risks while administering point-of-care and lab-based testing. Collaborate with physicians, case managers, and caregivers to manage referrals, provide patient education, and ensure clear documentation per Matrix guidelines. Perform visits lasting up to one hour across Sunday through Saturday, with flexible scheduling based on availability. Requires current NP licensure with prescription authority, board certification, and a valid driver's license.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
