Job Summary
Conduct adult and geriatric in-home health assessments, reviewing medical history, medication adherence, and social environments to identify care gaps. Administer point-of-care and lab-based testing as needed while collaborating with physicians, case managers, and family members on patient education and referrals. Provide services at member residences or skilled nursing facilities, ensuring clear documentation per Matrix guidelines. Schedule visits Sunday through Saturday from 8 am to 7 pm based on availability, earning $1,800 to $11,500 monthly per assessment completion. Master's degree, current NP licensure, and BLS/ACLS certification required with one year of experience preferred.
Required Qualifications
- Master's Degree or commensurate experience and satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Bodily Injury: $100,000 per person/$300,000 per incident
- Property Damage: $50,000
- Collision and comprehensive deductibles: $1,000 or less
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
