Job Summary
Travel to and from patients' homes to perform comprehensive, non-invasive physical assessments. Review medical history, conduct medical reconciliation for patient safety and compliance, and provide basic education on wellness and preventative healthcare. Ensure accurate and timely documentation of all interactions. This role offers flexible scheduling with competitive compensation and travel reimbursement for Nurse Practitioners and Physician Assistants conducting in-home Health Risk Assessments for Medicare and Medicaid members in Harrisonburg, VA.
Required Qualifications
- Master's Degree OR commensurate experience and satisfactory completion of NP/PA license
- Active State NP/PA license
- Active National board certification
- Current BLS certification
Desired Qualifications
- 12 months of experience as a paid advanced clinical provider
