Job Summary
Travel to and from patients' homes to perform comprehensive, non-invasive physical assessments for Medicare and Medicaid members in Brooklyn, NY. Review medical history, conduct medical reconciliation, and ensure accurate documentation while providing basic patient education on wellness and preventative healthcare. This role focuses on in-home health risk assessments without treatment or prescribing, utilizing dedicated scheduling assistance and comprehensive training. Compensation is $90 or $115 per completed assessment with a flexible schedule.
Required Qualifications
- Master's Degree OR commensurate experience and satisfactory completion of NP/PA license
- Active State NP/PA license
- Active National board certification
- Current BLS certification
Desired Qualifications
- 12 months of experience as a paid advanced clinical provider
