Job Summary
Nurse Practitioner or Physician Assistant (PRN) conducting in-home Comprehensive Health Assessments (CHA) during visits up to one hour to review a member’s health, medical history, medication adherence, social environment, and other risks. Responsibilities include adult/geriatric assessments, documenting per Matrix guidelines, ordering/administering point-of-care and lab testing, coordinating with physicians, case managers, social workers, and family members, and providing services at the member’s residence (homes or Skilled Nursing Facilities). Flexible scheduling (including Sundays–Saturdays, day/evening/weekend availability) with potential transition to Full-Time roles. Requires NP licensure and board certification in applicable adult-gerontology or family/applied specialties, current BLS/ACLS, 1 year NP experience preferred, and valid state driver’s license with insurance. Required Qualifications
- Master’s Degree or commensurate experience and NP licensure
- Current NP licensure in state of practice with prescriptive authority or ability to obtain
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
- Current BLS, ACLS or CPR certification
- 1 year of NP experience preferred (new grads encouraged)
- Valid state driver’s license
- Automobile insurance coverage as required by state of residence
- Strong computer skills and熟 familiarity with medical records software
- Excellent verbal and written communication skills
