Job Summary
Nurse Practitioner or Physician Assistant - PRN providing in-home comprehensive health assessments (CHA) up to one hour per visit for Matrix Medical Network. Responsibilities include conducting adult/geriatric assessments (medical history, diagnosis, treatment, health education), arranging physician referrals and case management referrals, ordering point-of-care and lab-based testing, collaborating with PCP and care team, and delivering services at members' residences including homes and Skilled Nursing Facilities. Flexibility to schedule across Sundays–Saturdays 8 am–7 pm with earnings tied to assessments completed and travel to members outside home locations. Sign-on bonus offered; education and licensure support available; requires NP licensure and related board certifications, BLS/ACLS, and valid driver’s license with insurance. Required Qualifications
- Master’s Degree or commensurate NP licensure
- Current NP licensure in state of practice including prescription authority or ability to obtain prescriptive authority
- AANP, AACN, or ANCC board certification in Adult/Adult Gerontology/Family/Emergency/Acute Care
- BLS, ACLS or CPR certification
- 1 year NP experience preferred; new grads encouraged to apply
- Valid state driver’s license
- Automobile insurance coverage for state of residence
- Strong computer skills and familiarity with medical records software
- Excellent verbal and written communication skills
