Job Summary
Conduct adult and geriatric health assessments in patients' homes or skilled nursing facilities, reviewing medical history, medication adherence, and social environments while administering point-of-care and lab-based testing. Collaborate with physicians, case managers, social workers, and caregivers to manage diagnoses, treatment plans, and referrals, ensuring clear documentation per Matrix guidelines. Provide patient education and follow-up support as needed during visits lasting up to one hour. Schedule availability between Sunday and Saturday from 8 am to 7 pm based on personal preference, with compensation tied to the number of completed assessments ranging from $90 to $115 per visit.
Required Qualifications
- Master's Degree or commensurate experience and satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Bodily Injury: $100,000 per person/$300,000 per incident
- Property Damage: $50,000
- Collision and comprehensive deductibles: $1,000 or less
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
